injection site beforehand. When that spot gets a somewhat warm feeling, you're good to get poked. You'll only feel a slight pinch at that point. If the dentist injects too fast, it'll hurt like a bastard regardless of topical being used or not. But, like I said before, get a new dentist at that point. If s/he injects slowly enough, your gums won't bubble out too much, and you won't feel it. Getting an injection in the palatal area pretty much hurts regardless, but that's not usually done for a simple cavity.
